| dc.description | El Síndrome de Munchausen por mandato, "un trastorno ficticio, por el cual la enfermedad del niño es inducida, promovida o provocada por la persona más próxima a él, generalmente su madre", es todavía mal conocido y su génesis imperfectamente comprendida. Esta comunicación está destinada a esclarecer al pediatra esta patología, con elevada morbilidad y secuelas, como así también de altísima mortalidad (9%). Se presentan cuatro casos clínicos disímiles, planteándose los aspectos clínicos de los pacientes y psicológicos de los padres, especialmente de la progenitora; y las pautas para su sospecha. "Uno de los atributos esenciales del médico es la disposición para aceptar la historia del paciente, y si hubiéramos de tener la mínima duda sobre la veracidad de sus afirmaciones, se debilitaría la estructura total de la medicina". El pediatra, habituado a pensar de ésta forma, al enfrentarse con el Síndrome de Munchausen por Mandato, deberá aguzar su experiencia, conocimiento y sagacidad para arribar a un diagnóstico certero. | - |
| dc.description | The Munchausen´s syndrome by proxy "a false disorder by which the disease of a child is induced, promoted or provoked by the nearest akin, most commonly his/her mother" is still poorly known and its genesis imperfectly understood. This paper is meant to enlighten the pediatrician about this pathology which entails high morbidity and after-effects, as well as an extremely high mortality rate (9 %). Three different clinical cases are presented, referring the patient's clinical features, the parent's psychological aspects, specially the mother's, and the patterns which allow the suspicion for the appropriate diagnosis. "One of the essential attributes of the physician is his disposition to accept the patient's background and if we could have the smallest doubt about the truth of his statements, the global structure of medicine could be weakened". The pediatrician, used to think this way, when is faced to Munchausen´s syndrome by proxy should be sharp his experience, knowledge and cleverness in order to get an accurate diagnosis. | - |
